When you choose Revive Balance, our only focus is you. To ensure this happens and that we maximize your time, please fill out all forms and questionnaires prior to being seen. We also ask that our clients arrive on time because your time cannot be extended. Please provide us with at least 48 hours' notice if you need to change, reschedule or cancel your appointment. If you cancel within 24 hours of your scheduled appointment, you will be charged 50% of your service. No-show will be charged the full price of their scheduled service.
